Who needs a DVLA medical
Anyone applying for or renewing a Group 2 (category C or D) licence needs a D4 medical examination report. One is required for a first application, then at renewal from age 45, every five years to 65, and annually from 65. Drivers who develop a notifiable medical condition may need an assessment at any point, whatever their licence category.
What the examination covers
The D4 form is set by DVLA. It covers visual acuity and visual fields, diabetes and its treatment, heart conditions and blood pressure, neurological conditions including epilepsy, blackouts and stroke, sleep apnoea and excessive daytime sleepiness, psychiatric history, alcohol and drug use, kidney and general health, and current medication.
What to bring
- Your DVLA D4 form, if DVLA has sent you one
- Photo ID
- The glasses or contact lenses you drive in, and your most recent optician prescription
- A list of your current medication, and your GP practice details
- Specialist letters or monitoring records if you have diabetes, a heart condition, sleep apnoea or a neurological condition
